Work and Access Application Forms

The updated WA1 and WA2 application forms, found in the appendices of the operational guidelines. The WA1 is for individuals and self-employed people applying for Communication Support/ISL, Personal Reader, or Work Equipment. The WA2 is for employers or self-employed people applying for Workplace Needs Assessment, In-Work Support, Workplace Adaptation, Disability Equality and Inclusion Training, or Communication Support/ISL in respect of a Workplace Needs Assessment and/or In-Work Support.
